Our Partnership

The Surrey Heartlands Health and Care Partnership brings together health and care organisations, and wider partners, covering a population of around 1.1 million people.

Called the Surrey Heartlands Health and Care Partnership, we are formally known as an ‘Integrated Care System’ (or ICS for short). That means we take collective responsibility for improving the health of the local population, managing resources (including money) and making sure services are high quality.

The four overarching aims of bringing partners together in this way are to:

  • Improve outcomes in population health and healthcare
  • Tackle inequalities in outcomes, experience and access
  • Enhance productivity and value for money
  • Help the NHS support broader social and economic development

You can read more about the definition of ICSs and what they have been set up to do on the NHS England website.

Our partnership includes a range of organisations, including the NHS, local government, social care providers, hospices, social enterprise partners, Healthwatch Surrey and the wider voluntary, community, faith sector and charities, bringing us closer together so we can focus on the wider causes of poor health - things like pollution, poverty, housing and education.

Surrey Heartlands coverage with regional boundaries

Our partnership covers most of Surrey as shown in the map below. The rest of Surrey (including the borough of Surrey Heath and parts of Farnham) are covered by the Frimley Health and Care system.
