Blood pressure at Home monitoring service
High blood pressure (hypertension) is a major cause of heart attacks and strokes and is one of the top three health conditions in Surrey.
Blood pressure at Home (BP@Home) is a service which enables patients with high blood pressure (hypertension) to manage their blood pressure more effectively from the comfort of their own home.
Managing your blood pressure remotely means that you won’t have to visit your GP practice so often, saving both you and your GP practice time, and putting you at less risk of catching covid-19, colds and flu.
It will also help you understand and manage your condition better and take steps to improve your health through making simple lifestyle changes such as changing your diet, increasing your exercise or reducing smoking.

Participation
Your GP or Clinical Pharmacist will identify whether you would benefit from using the service and if so, a member of the BP@Home team will invite you to participate in the service.
If you are invited to join the blood pressure monitoring service, you will be able to take your blood pressure yourself using your own blood pressure monitor, you can then submit your blood pressure readings directly to your GP practice using either a digital app, following a link from an email or text message, or directly over the phone with the help of our BP@Home team.
